1. No clearly defined problem Teams jump into tools (5S, Kanban, VSM) before agreeing on what they are actually trying to fix. Without a sharp problem statement, Lean becomes an activity without impact.

2. Lack of leadership commitment Lean is not a side project for the operations team. If leaders do not walk the gemba, remove obstacles, and model the behavior, the initiative stalls within months.

3. Treating Lean as a toolkit, not a mindset Tools deliver short-term wins. Culture delivers lasting results. Skipping the people side, coaching, problem-solving habits, respect for the worker, is the number one reason Lean transformations fail.

Get these three right, and the rest becomes execution.
